Note for Parents and Athletes:

We understand that situations can arise that may require you to see a physician before coming to the training room. Once a physician is seen for a condition that may affect athletic participation, we are required to have a note for release because your care is under that physician. Please assist us in providing a note as soon as possible as well as any subsequent notes that may help in the continuity of care for an injury or illness. Even if the physician does not explicitly state that there are any restrictions, we are still required to have a note before any participation may resume.
