Training Room Medical Forms

Athletes who see a physician or are referred to a physician, must provide a note to the athletic trainers BEFORE they will be released to any participation. This must include their restriction(s), if any. Even if the physician says there are no restrictions, we must have documentation of this. Additionally, the paperwork must include a diagnosis. Any form may be used to obtain this information, however, we highly encourage the use of the forms below as we have streamlined the information that is needed. If there is information missing, it may delay the ability to return until that information can be obtained. This is applicable to any illness or injury.

We highly encourage all individuals to submit any of these notes to ghstrainers@georgetownisd.org so that they can be added to our RankOne portal.

In this section you will find a variety of different forms including flyers we may recieve from vendors and coupons for discounted physicals. Because promotions change, we always recommend reaching out to clinics to ensure that the promotion is valid and to see if an appointment is necessary.
