COVID Green Bay testing site information:

CVS to be free you must have a state ID ( Drivers license, state ID or Social security number) otherwise they test is charged to the patient

Walgreens 100% free without insurance, or ID, they just need to make appt prior on the app/website and not enter insurance info and NOT be seen in the care.

Prevea 100% free if set up through the Prevea Website or app create an account if one hasn't been made prior. New User Account. If no SS enter 999-99-9999 OR call (920) 272-3540. Do not schedule an appt to see MD or be seen in u/c or ED or charge will be incurred.

Bellin also 100% free, sign up via their web site New user account or call 920-445-7313. Do not schedule an appt to see MD or be seen in u/c or ED or charge will be incurred.

To sign up with a new user account:

1. The easiest way to sign up someone for mychart that has no SSN documented, is to have them call the COVID hotline (920-445-7333) and then have us send them an email link. In this email link they only require DOB and a security question to finish the sign up process.

2. Or based on the call they can just have the nurse ask questions about testing and sign them up for a location, date

and time.

3. They can obtain an authorization code to access still on website without a SS# but if you have to call they just need to identify they need it for work or school and they will do this test. This also applies under the symptom checker…..Molly told me the last question was if you hit “you need it for work or school it allows you to get the test”

Arrival at facility for COVID test:

1. At the testing site:

a. Staff will ask for full name and birthdate to confirm identity

b. Staff may ask for an identification, simply to confirm spelling, but an ID is never required



You may have COVID-19 if you have any of these symptoms:

  • Cough (new onset or worsening of chronic cough)

  • Shortness of breath

  • Fever

  • Chills

  • Sore throat

  • Runny nose

  • Muscle or body aches

  • Headache

  • New loss of taste or smell


Not everyone with COVID-19 has these symptoms. For many, symptoms are mild, with no fever. Some people may also experience fatigue or gastrointestinal symptoms such as nausea, vomiting, diarrhea, or abdominal pain. You can still spread the virus to others even if you have mild or no symptoms.

If you have symptoms of COVID-19

  • Stay home and isolate yourself from others.

  • Notify the school.

  • Get tested.
