Providing transition services is a collaborative effort between the youth, parents, school staff, and agency personnel. Agencies may provide or pay for transition services while the youth is in high school and into adult life. Agency involvement should be considered for all youth with disabilities and parent permission must be obtained. Agency involvement should be detailed in the youth's IEP.
