Work Permit Application

Below are the directions for obtaining a work permit:

• Students can find a Work Permit Application by stopping by the High School Counseling Office (or click on the file to the right)

• Student should complete the application, making sure to fill in all required information and that the application is signed by their parent/guardian.  The completed application and the Physical Fitness Certification page signed by the student’s physician should be brought into the High School Counseling Office BY THE STUDENT.

• Once Mrs. Thompson has confirmed that all documents are filled out correctly and are signed by the appropriate people, she will issue the work permit.

• If a student was issued a work permit previously, but has lost it or a previous employer will not return it to them, Mrs. Thompson can issue a duplicate work permit. Contact Mrs. Prorok as directed below.
