参加登録フォーム（登録締切日：7月31日（月））

　申込数が当初の予定上限129名に達しましたが、多くの方に参加していただけるよう申込数上限の拡大に尽力いたします。今後ご登録していただく方の参加可否は宿泊施設の予約状況によることとなりますが、参加をご希望される方は諦めずにご登録をお願いします。

　※該当する方には、本会メールアドレスより受け入れの可否を7月31日までにメールでご連絡いたします

登録フォームでは以下について入力していただきます

・氏名、連絡先などの基本情報

・口頭発表の希望の有無、発表分野、発表時間（5分、10分、15分から選択; 質疑含む）

　※発表のタイトルと要旨（200字程度、400字以内）は後日提出していただきます

（提出締切日：8月15日（火））

・アレルギーの有無

・キャンセルポリシーへの同意

・ひとこと

Registration Form (registration deadline : July 31st)

　The number of applications has reached the initial maximum limit of 129 participants. However, we are making efforts to expand the limit to accommodate more people. The confirmation of participation for those who register from now on will depend on the availability of accommodation. If you wish to participate, please register without giving up.

※Those affected will be notified via email from our organization's email address regarding acceptance or rejection by July 31st.

In the registration form, we kindly ask you to provide the following information: 

・Basic information such as name and contact details

・Preference for oral presentation, field of presentation, and presentation time

　(choose from 5 minutes, 10 minutes, 15 minutes; including Q&A)

　　※The presentation title and abstract (around 100 - 200 words) will be submitted at a later date

　(submission deadline: August 15th)

・Allergies (if any)

・Agreement to the cancellation policy

・Brief comment
