INSTRUCTIONS FOR REGISTRATION FORMS
Click on PAY (bottom of page on ITEMS tab)
a. If you raised enough money through Vertical Raise, skip this step
b. If you are buying out the fee or are doing fundraising+buy-out, put the amount that brings the total to $200
c. If you are planning to request a payment plan, contact Ms. Deussen
d. Post-payment, $175 goes to our Booster Club, who then distribute funds as needed. $25 goes towards registration under the National FFA, processing fees incurred by the school bookkeeper, and other similar items. All students will raise/pay a minimum of $200 to cover the expenses.
Click on FORMS tab
a. If you did not do the pay step, it will ask if you are sure you would like to proceed. Click CONTINUE
Fill out the five forms and waivers.
a. Parent signatures and information are needed, students should NOT complete without a parent present
b. Read all information and ensure you understand the content. You and your student can and will be held accountable for promises made and conduct standards that agreed to by signing.
b. For the Medical Information form, you will need to know your health insurance details
c. Forms can be edited after being filled out, but will not autosave progress
Why does Guyer FFA need...
My health insurance?
FFA members have lots of opportunities to travel for competitions and events. Advisors, in emergency settings, need information to best help your student get help. Advisors will attempt to get in contact with the parent/guardian in the event of medical distress, but their priority is keeping students safe.
To know students' "special medical conditions, allergies, or special diet requirements"?
The advisors and officers want to make the Guyer chapter inclusive, engaging, and safe for all of our members. The information is confidential and shared on a need-to-know basis so we can best accommodate all members.
To collect dues?
FFA is not a school-sponsored organization. Member fees go towards meeting equipment, competition entry fees, travel, gas, and scantron fees, supporting chapter events like the Barn Shower, Ag Awareness Week, and the Banquet, purchasing project and competition materials, and Offering opportunities in leadership development, career exploration, scholarships, and community service.
