FISD Medication Policy: https://www.friscoisd.org/o/fisd/page/medical-services
You must bring the age-appropriate medication to school in the original container labeled with child's name. Per FISD policy, students may not bring in their own medications. The parent or guardian must bring the medication to school. Expired medication will not be accepted. Request for Administration of Medication form has to be completed by parent/guardian and signed by a doctor, if required: https://aptg.co/yrjYvy
Over-The-Counter: Tylenol (acetaminophen), Motrin/Advil (ibuprofen), Benadryl and Tums are the ONLY FISD-approved medications. Any other medication, including over-the-counter medications, will require a doctor's signature (this applies to field trip meds as well). The nurse may give up to 10 doses, and then a doctor's signature will be needed in order to continue to give the student the medication. A Parent/Guardian Request Form has to be completed by a parent: https://aptg.co/yrjYvy Each form can hold 2 medications; use additional forms as needed.
Prescription Medication: Must have an affixed prescription label with the student's name on it. We are only able to administer prescription medications if they require administration specifically during school hours. A Parent/Guardian Request Form has to be completed by parent: https://aptg.co/yrjYvy Each form can hold 2 medications; use additional forms as needed. Each prescription should have an affixed prescription label including the student’s name, name of drug, date the prescription was filled, and directions concerning dosage and duration of treatment. If a label is not available, the medication must be in the original container, and a written prescription signed by the doctor that includes the above information will be accepted.
ALL medications must be kept in the nurse's office. The only exception is certain asthma medications and Epipens and ONLY if a doctor has completed a form allowing them to do so.
The school does not purchase or provide medication for your child.
