5th Grade Camp

CAMP DATES: 

November 8th-10th, 2023

Dear 5th Grade Parents,

I wanted to take this opportunity to go over the FISD medication policy for this year and how it impacts your child at camp.

The policy states that the following over the counter medications may be administered by the nurse but must be provided by the parent along with a signed FISD consent form.  These medications are: Children’s Tylenol, Children’s Advil (or Children’s Ibuprofen), Tums, Benadryl.  If you think your child may need any of the approved over the counter medications, please send it in its original container/box labeled with your child’s name.  If you plan to send more than one, please place them in a Ziploc bag with their name clearly labeled on the bag.  If you plan to send ANY other over the counter medication (i.e., Zyrtec, Claritin, Mucinex, Adult-strength formulations of Tylenol or Ibuprofen products, etc.),  your DOCTOR must  provide me with a note stating the dosage and frequency to give the medication, along with your child’s name.  Your doctor may also fax that information to the clinic at 469-633-2284. Please don’t put this off until the last minute.  It usually takes several days for doctors to return paperwork to me.  If I don’t have all of the paperwork I need, I will not be able to bring the medication to camp or administer it to your child.

If you plan to send ANY prescription medications, please fill out the attached form completely indicating the time you give the medication each day at home.  When sending any medication, please check the expiration date.  I am unable by law to administer any expired medications.  I am also unable to administer any prescription medication that was not prescribed for your child {like a sibling’s inhaler, for example}.  All prescription medications must be in their original container with the prescription label attached to either the medication itself or the box/container.

If you plan to send any medications whether it be over the counter or prescription with your child to camp, you must fill out the Camp Medication Form in its entirety.  This provides me with helpful information when planning administration times.  

IF YOU ARE NOT SENDING ANY MEDICATIONS TO CAMP:  Please write your child's name at the top of the Camp Medication Form, then write "NO MEDS SENT TO CAMP" across the form and sign and date the bottom.  Failure to do this will result in the form being sent back to you to complete and return before we can finish your child's camp registration, as your child's teacher and I will think you just missed the form in the mountain of camp paperwork.  

If your child suffers from asthma, I strongly suggest you send an inhaler, even if they haven’t had any asthma issues lately.  We will be outside in the woods for 3 days, and environmental and weather triggers for asthma and allergies are very strong. I would much rather have asthma medication with me and not need it than need it and not have it.  Nobody wants to have to go the ER for breathing treatments when they're supposed to be having fun at camp!

If your child currently has any medication in the clinic and I have paperwork on file, I will be taking those medications with me.  You will not need to fill out any additional paperwork for those medications only. 

If you are going to be chaperoning our trip, you can bring all of your child's medications and administer them to your own child as you see fit {but not to any other Liscano student}.  Just fill in your child's name at the top of the Camp Medications Form, then write "PARENT CHAPERONING" across the form and sign and date the bottom.

LAST DAY TO DROP OFF MEDICATION FOR CAMP: 

 I will not accept medication after the Friday before camp (11/3/23) due to the amount of preparation I am required to do for each student.  Please plan accordingly.
