LETTER OF INTENT AND PARENTAL/GUARDIAN PERMISSION FORM for MAGNET PROGRAM ACCEPTANCE

MAGNET PROGRAM: _________________________________________________________________

STUDENT’S FULL NAME: __________________________________________________________________________________

PARENT/GUARDIAN’S FULL NAME: ________________________________________________________________________

I acknowledge that I am fully aware of the district rules and procedures listed, below. By signing this form, I indicate my acknowledgement of these rules and procedures.

FULL PARTICIPATION: Any student accepted to a Magnet Program in the Freehold Regional High School District will enroll in the high school in which the program is located. Students will not be permitted to attend a Magnet Program for less than the full course load for that program.

VOLUNTARY WITHDRAWAL: Voluntary withdrawal from a program may only occur at the end of a school year. Parents must give written notice of a student's intent to withdraw from a Magnet Program. If a rising 12th grader leaves a program, the student will retain his/her class rank from the previous three years. Students will be required to return to the high school that they would have attended if they had not been accepted to the Magnet Program. Parents of rising 11th and 12th graders, however, may appeal in writing to the Principal of the program school for their child to remain in that school rather than return to the home high school. If a request for a rising 11th or 12th grader to remain in the program school is approved, the district cannot guarantee that transportation to the program school will be available. Parents may need to provide transportation to/from school. Transferring to another FRHSD program is not permitted (e.g. another Magnet Program or the International Baccalaureate Program.)

ADMINISTRATIVE REMOVAL: Students who have committed to participate in a Magnet Program in our district are expected to maintain good grades, good attendance and good decorum. A decision to terminate a student's enrollment from a Magnet Program will not be made lightly. The decision to remove a student will be reviewed and approved by the high school Principal in consultation with the supervisor of the program.

The following are grounds for terminating a student's placement in a Magnet Program:

1) Failure to maintain an overall “C” (2.3 GPA) average in all courses including a “C” or better average in each program

course.

2) Withdrawal of credit in any class due to excessive absence as per the Freehold Regional High School District Board of Education Policy and Regulation 5200: Attendance.

3) A serious infraction of the student discipline code carrying the penalty of suspension from school with the recommendation of the Principal for removal from the enrolled program.

4) Academic dishonesty as outlined in Freehold Regional High School District Board of Education Policy 5701: Plagiarism.

5) For the Business Administration, Computer Science, Medical Sciences, and Science and Engineering programs, failure to show completion of Algebra I with a grade of "B" or better on the 8th grade transcript.

Once removed, the student will be transferred to the school that the student would have attended if he/she had not been enrolled in the Magnet Program. Transferring to another FRHSD program is not permitted (e.g., another Magnet Program or the International Baccalaureate Program).
