Step 2
Indicate your interest to be vaccinated through your respective LGU based on the address of your:
Permanent residence, or
Current residence, or
Workplace
Go to the LGU Vaccination Process pages then select your LGU to see the registration steps you need to follow.
Step 3
Prepare your FPS ID and COE.
Certificate of Employment will be issued to you for vaccination purposes upon request. Please send a ticket request to Paycom AskHere.
For employees hired during the WFH transition and not issued with a physical FPS ID, you may submit a Digital ID request to Paycom AskHere.
Persons with Comorbidities
When is a person's comorbidity considered to be controlled?
Persons with controlled comorbidities are those who have no symptoms of their comorbidity, have stable vital signs (heart rate, respiratory rate, and blood pressure), have had no attacks, admissions, or changes in medication in the past 3 months, and are currently not hospitalized.
Where can persons with the identified comorbidities get their Medical Clearance?
Medical clearances can be obtained from attending physicians through teleconsultation, consultation at designated facilities, or Rural Health Units (RHUs) or other primary care centers designated by LGUs. LGUs shall ensure that mechanisms to provide medical clearances to persons in A3 priority group are accessible and available to all who need it.
"Economic frontliners" include: private employees required to physically report to work, employees in government agencies, workers in the informal sector and self-employed who may be required to work outside of their homes and those working in private households
