1. Call Maxicare's 24/7 Teleconsult Service based on your location.

2. Inform the customer care rep of your reason for consultation (other Maxicare details may be asked). The rep will then find an available physician based on need and will process a Letter of Authorization (LOA) request.
3. Wait for an email from Maxicare with the PDF copy of your LOA and the assigned physician's contact information as well as the consultation instructions.
4. Contact the physician following the scheduled consultation.
5. Proceed with the consultation and inform the physician about your illness.
6. Request for:
a. Medical Clearance
*Medical clearance for the vaccine is not required except for immunocompromised individuals such as those with autoimmune disease, HIV, cancer patients currently undergoing immunosuppressive therapy, transplant patients, those undergoing steroid treatment, and patients who are bedridden or with a poor prognosis.
b. Medical Certificate or Prescription with a notation of the diagnosis or other medical advice

Steps to install and set up:
1. Download Doctor Anywhere app
2. Sign up using any of the following:
a. Facebook
b. Google
c. Apple
d. Email
3. Go to Profile > My Info > Corporate Insurance/Benefits
4. Click Continue
5. Select Maxicare as your corporate insurance/ benefit program
6. Enter your Maxicare Card Number and Date of Birth
7. Enter your mobile number
*You will receive a PIN.
8. Read the Terms & Conditions and check the box if you Agree.
9. Go back to Profile > My Info to complete all other information (e.g. ID, DOB, Address, etc.)
Steps to consult:
1. Open Doctor Anywhere app
2. Tap Video Consultation
3. Select intended service
a. General Practitioner
b. COVID-19 Advisory
4. If General Practitioner, tap Search for a GP
5. Choose a General Practitioner from the list and tap Select
5. Select a Date and Timeslot from the Calendar
6. Tap Confirm to continue with the video schedule or tap Select another time
*You will get an app notification of your scheduled consultation.
