To allow more Dabawenyos to have their names listed in the ongoing vaccination program of the Davao City government, they may now pre-register online using the SafeDavao QR mobile application scanner or website.
Individual users of the SafeDavao QR or DQR may now start filling out information for the COVID-19 vaccine pre-registration, which has started Tuesday, May 25.
Users may pre-register through their SafeDavao QR mobile application scanner or the website, profiles.safe-davao.com.
Upon entering the registration site, users must click on the “Register Here” button where they will be directed to log in their DQR code text, mobile number, and date of birth.
Registrants are reminded to read all information necessary for registration before answering the questions.
After completing the online registration process, the user must wait for the announcement of their assigned schedule from the district health office or through the DQR push notification.
Individuals who are part of the A1 and A2 priority groups, which refer to healthcare and frontline workers, and senior citizens, respectively, who have already pre-registered, need not undergo the online pre-registration using the SafeDavao QR.

Option A: Manual Registration
The steps to be followed for Manual Registration are as follows (UPDATED June 1, 2021):
1. Make sure that all the requested details in the form are answered.
2. Submit the understood forms to your barangay / Homeowner’s Association (HOA).
3. Include one (1) 2 x2 size ID photo and photocopy of any government-issued ID with address.
4. Wait for a text message from Ezconsult. Your PATIENT ID NUMBER and other relevant information will be revealed.
5. Bring a government-issued ID with you on the day of your vaccination.
Option B: Online Registration
Registration can also be done online with the following steps:

2. Answer the questions and make sure your details are correct.
3. Your information will be verified. Wait for a text message.

Second Dose Reminders:
Refer and follow the second dose schedule indicated at the back of your vaccination card.
Your second dose will be administered in the same vaccination site where you had your first dose.
Be at the vaccination site 15 minutes prior to your schedule.
Starting July 9, 2021, tamper-proof security seals will be affixed on the vaccination cards following the second dose to ensure that fake cards are not mistaken for authentic ones. An authorized representative of Task Force Vax to Normal will also sign the vaccination card right beside the hologram seal.
