ATTENTION PARENTS OF INCOMING 6TH GRADE STUDENTS:
Letters went home via school messenger, and can be found under the Health Forms section about the immunizations that are REQUIRED for your child to attend 6th grade. These forms need to be filled out by the pediatrician and returned to the school nurse BEFORE your child may return to school in September. If you have any questions please email me as I will be checking my email periodically during the summer.

Medication in School
Any medications that were in school have been returned to the child in a large envelope. BEFORE the child returns in September the enclosed paperwork needs to be filled out by both the pediatrician and parent and the new appropriately labeled medication needs to be returned to the nurse's office. These forms can also be found under the Health Forms section. Any questions please email me as I will be periodically checking my email throughout the summer.
Physicals
Physicals
It is reccommended that your child receives a physical at least once during the Pre Adolescent Stage (Grade 4-6). If you child does not already have a physical form filled out for this period, please have their pediatrician fill out the form and return it to the nurse's office. The Physical Examination form can be found under the Health Forms section. If you have any questions please email me as I will be checking my email periodically during the summer.
