Any student who needs to be excused from Physical Education, requires the use of crutches, the elevator, has a cast or brace must have the Physical Education Exemption Form filled out completely by the doctor.
Student-athletes participating in ANY school-sponsored sport must have a full physical completed within 365 days and submit the sports physical forms BEFORE participating. If the student-athlete's physical is more than 90 days old, a Health History Update Form MUST be filled out.
Any student entering the Fort Lee School district must have a complete physical within 365 prior to entering school.
As per N.J.A.C. 6A: 16-2.1, 2.2 and pursuant to N.J.S.A. 18A:40-4., the Fort Lee Board of Education advises parents of the importance of students obtaining a physical examination at least once during each of the developmental stages:
Early childhood (kindergarten through grade 3)
Pre-adolescence (grade 4 through 6)
Adolescence (grade 7 through 12)
Students who require medication to be administered in school must have a physical within 365 days.
Students with chronic illness must have a physical within 365 days
As of September 2008, all students who are enrolled in grade six in a New Jersey school MUST receive a booster dose of Tetanus, Diphtheria, and acellular Pertussis vaccine (commonly known as Tdap), as well as one dose of Meningococcal vaccine, given no earlier than the 10th birthday, before they can attend school.
