If the reason is due to "FOB-Not Given" use "Missing FOB" in the jotform.
This should ONLY be filled out for DP's that are Voluntarily Deactivating their account!
This should be filled out when a DP had feedback to give on the WH
This will be filled out for EVERY successful package pickup return
This jot form will be filled when feedback needs to be given back to CX for an incorrect reach out to DS.
This jot form will be filled out when there are any inaccuracies with the Gladly Voice Summary
This jotform will be completed for FOBs that have not been returned within 24 hours.
Note: Please be sure to review the FOB Return payout sheet. .If the DP has two previous payouts for FOB returns, we will not offer the payout.
