The Campus Intervention Team must meet to determine whether or not the data leads to proceeding with an evaluation. Click here for the full process.
Complete the steps in the flowchart then complete the form.
Document the following:
What happened medically or academically since the last evaluation that would warrant another evaluation?
Was the evaluation on file conducted utilizing best practices? (Review with Director of Special Populations) Did the student in question have any cognitive deficits?
What is the student’s responsiveness to the supports currently in place?
Contact Director of Special Populations with any additional questions or concern.
An initial referral should be deemed appropriate when:
-Determined an educational need by the Campus Intervention Team (CIT)
-Determined an educational need by the Campus 504 Committee
-Parent Request
-Teacher Request
Collection of Information prior to Consent for Initial Evaluation will be handled by the campus designated general education staff member. In many cases this is the Campus Counselor.
