Online Forms

Instructions:

Log in to Praxi Parent Portal

Click the three lines (menu) on the left of the House Icon

Scroll down and click Online Forms

Choose a Student from the drop down field

Choose a Form from the drop down field

Click Start New Form

Log in to Praxi Parent Portal

Click the three lines (menu) on the left of the House Icon

Scroll down to click Online Forms

Choose a student from the drop down list

Choose "Student Re-enrollment Form"

Click Start New Form button

Fill in missing information/verify the auto-populated information
      When you are done, e-sign and date

Click Finalize & Submit in the form menu

(This will need done for each student.)

Registration and book fees need to be paid before the application will be accepted.

Fees per student: PreK - includes book and registration for $150 total

                                K-12 - books are $200 and registration is $50 (thru June 30th)

Payment can be made using Paypal with the link within the form, the Praxi announcement page or here.

It can also be taken to the office in person or mailed: FBCA, 301 Mary Street, Rosamond, IL 62083

A paper version of the registration form, along with all other forms needed for school records can be located on the Documents & Forms Site.

New students that are siblings to current students can be registered using the New Student Sibling form on the Parent Portal - Online Forms page.  Please only use this if the sibling has not attended FBCA yet.

Spread the word that any new families wishing to join the FBCA family can also register online by going to the school website (fbcafalcons.org) and clicking the Admissions page.
