Forms

2024 Forms:

Complete the “Forms Checklist” (See attachment below) and follow the instructions below and on the checklist. All items on the checklist need to be completed and/or turned in by Friday, March 15th. If a player does not have their items in by this date, they will not be able to participate in school baseball for this season.

• 1. All players are required to pay a $50 activity fee, unless otherwise arranged with Mr. Ohlhauser (Davies Activities Director). If you do not make the team, your fee will be reimbursed.

  2. All players are required to have all physical forms/Form B on record with the school. This must be done annually.

  3. All players are required to complete the forms on the checklist below. Please do with your parents/guardians.

These forms will be given out once you are informed of which team you have been placed on. Please return these forms filled out and with the payment (Davies Baseball) to Coach Anderson on Monday, March 25th.

Please complete the form below if you are new to the high school program or if your information has changed. This helps the coaches have accurate contact information for the player and parents/guardians.
