GLA Requirements & Paperwork
Pre-Departure Requirements
- Flight Itinerary
Once approved to book your flight, please provide us a copy of your roundtrip booking confirmation.
Requires a doctor's signature. Your doctor's office can fax it directly to us (info listed on the form) or you may submit it to us.
- Copy of ID(s)
International Travelers: Passport - Clear picture or scan of your valid passport photo page. We recommend 6 months validity beyond your return date, minimum.
Domestic Travelers:
1. Copy of ID - whichever ID will be used to travel & check in with the airline / TSA
2. Copy of Health Insurance Card (front & back)

This can be paid with Visa or MasterCard in your online account, by direct deposit, by mailing us a check, or by calling our office at 858.771.0645
To submit the above paperwork, you can either email items to travel@experiencegla.com or upload them directly to your online account. Please note that submitted documents can take several days to process.
A $50 late fee may be assessed if all pre-departure documents are not obtained and submitted by your due date! Please see your Welcome Email or your GLA Dashboard for the due date for your program. Please contact us as soon as possible if you foresee a delay!
Parental Authorization Form - For Minors (17 or younger)
Parental Authorization Form - If your student is a minor (17 or younger), we recommend that parents complete two original notarized copies of this form. Many times it's not checked by airlines or authorities, but sometimes it is. We know for sure it is needed for Belize, Ghana (see Visa packet), and South Africa (additional minor forms required). It is a best practice whenever a minor is traveling without parents to ensure there are no hiccups and so anyone who needs to verify knows the minor is in a safe and approved situation. Be sure your student keeps this form in their carry-on luggage! GLA does not need a copy of this form. See Sample Form Here! IMPORTANT: The two original copies of this form should be notarized. You can visit your local notary or use an online notary to complete this step.
Visa or Travel Permit Requirements
Students traveling internationally from the US may need additional paperwork for certain locations. Please see program-specific info for details:
Visas and Passports can be processed through G3 Visas. More info about their services here.
- An Entry Visa is required for entry for US Passport-holders for the following countries:
South Africa (special requirements for minors)
Other Documentation - Students traveling on a non-US passport or with complex citizenship or immigration statuses may require additional documentation to travel. Returning to a country other than the US can also affect the types of forms you will need. Please contact us or your local embassy if you are unsure. We also recommend you speak with G3 Visas at global@g3visas.com who can assist.
Bali Participants: Please bring original completed Scuba forms with you to Bali for the diving experience.
Other Useful Forms
Contact card - Fill it out with the information found in the "Staff & Emergency Contact Info" and bring it with you when you travel. Please note, this information may not be fully available until spring.
Roommate Request Form - If you'd like to request a roommate, this must be completed by both families at least 2 weeks before the program. (rooms are based on gender identity)
Scholarship Application - Instructions/forms must be submitted to apply for a scholarship.
Medical Form - As part of the online application, a detailed medical form was completed. If there are any changes to medical details, please notify us immediately.
