We are Alyssa Fuggiti, and Elizabeth Deming, and we will be (or continue to be) your child's speech and occupational therapists this year. Ultimately, the goal of school-based therapy is to support your child's access to their general education. We wanted to let you know that in order to determine what your child may need, our plan for the first few weeks of school will be working in the background- reviewing your child's evaluation and educational plan, consulting with their teachers, and reaching out to discuss your child's strengths and needs (especially noting how the past few months have looked so different than your regular routines).
Once we have gathered that information, we can begin building regular weekly schedules. Therapy may be implemented in one or more of the following ways:
live virtual meetings with us, your student and one or two other children with similar goals
weekly on-demand lessons posted to your child's google classroom
our participation in your child's general education class live videos or lessons to provide support
IMPORTANT NOTE
In order for these services to be provided, we will need a signed teletherapy consent form from each family. Please see the attached document. If your child receives more than one support service (e.g. physical or occupational therapy, only ONE consent form is needed).
You can return this form in one of the following ways:
open in google docs, or microsoft word
add your name, your student's name, and an "x" to the consent line and email back to either email address below.
print the document out, fill it out, scan (or take a picture of it) and email back to either of us
come into the Mill Plain front office during regular office hours and ask for a paper copy to sign
request a paper copy to be mailed home to you to mail back or drop off at Mill Plain front office
schedule a time for a phone call with one of us and give your verbal consent over the phone
