Transition Assessment
The purpose of Transition Assessment is to collect data and assist individuals with disabilities in making informed choices regarding education and training, employment, and independent living. Assessments should identify a student’s strengths, preferences, interests, and needs.
Information from the assessments, whether formal or informal, is used to develop the transition plan, including annual transition goals, postsecondary goals, the course of study, and the coordinated set of activities.
All roads lead to improved postsecondary outcomes.
