FINAL FORMS
All players must be registered and cleared through the FINAL FORMS website before the first day of tryouts.   

SPORTS PHYSICAL FORM
All players must have a current physical on file at Peninsula High School before August 22nd to be eligible to tryout. 

ASB CARD PAYMENT

All athletes are required to purchase an ASB card. If you need assistance, please contact the athletic department at 253.530.4411. You will need to know your athletes STUDENT NUMBER.

PHSVB PLAYER & PARENT HANDBOOK
Please read through the PHSVB Parent & Player Handbook. It is lengthy, but imperative you read through this together as parent and athlete before tryouts to understand our program!

All athletes in the volleyball program will be required to wear PHS practice shirts to practice and PHS sweats to all matches. All athletes are required to purchase a practice t-shirts paid through the bookkeeper.

Sweats will be issued to players. There is a $100 replacement cost if athletes lose or damage their issued sweats.

Optional Purchases through BSN Team Store:  
Additional spirit wear will be available to purchase through our Online Team Store. Team Store purchase will take four weeks for delivery. 

TRANSPORTATION RELEASE FORM

Players are expected to ride the bus to and from all competitions with their team. In the rare circumstance that they need to ride one way with a parent to or from a competition, they may fill out the transportation form. It must be approved through the athletic department at PHS THE DAY BEFORE THE event and given to your coach.

Picture Day:  

Please make sure you have your uniforms, volleyball shoes, clean knee pads, black spandex and white socks! You will wear black spandex with your uniforms on game days. Here’s the order link: Picture Order Link. I know this is short notice, but orders placed by tomorrow will receive free buddy pictures, so don’t miss out!

With the increased ability to work with our programs through the summer, we will also have an increase in encountering incoming freshmen who DO NOT have ArbiterSports coverage. We do not have a mechanism in place to clear these students for summer participation outside of the “Open Gym Agreement”. Please find this document attached. You will need to have copies of this document ready in case new students arrive at your summer workouts. I am also attaching the weight room release for those programs that anticipate using those facilities.
