Please submit the completed transcript request form to Ms. Ilene Camillieri in the Counseling Office or email it to icamillieri@edgemont.org
NOT required if you and your parent/guardian signed an online agreement through the program in which you are applying
Please note that this form is required in order for your counselor to complete a recommendation
