Transcript Request and Recommendations

Please see below for the required forms for a transcript and/or counselor recommendation request for an outside program.  AT LEAST three weeks notice is required so please plan accordingly!

 Before submitting this form, you must email and/or meet with your counselor to ask for a recommendation and send all necessary information.

Transcript Request Form

Please submit the completed transcript request form to Ms. Maggie Cook in the Counseling Office or email it to mcook@edgemont.org 

NOT required if you and your parent/guardian signed an online agreement through the program in which you are applying

Recommendation Form

Please note that this form is required in order for your counselor to complete a recommendation
