All band students must have these forms completed by their stated deadlines.

Please print out this form, sign it, and return it with your student by August 29, 2025.

2025-2026 Legacy Middle School Band Handbook Signature Form

Legacy Middle School Band Shirt Order Form 

(Due by August 29, 2025)

Each student will need the official 2025-2026 Legacy Middle School band t-shirt.

Please download the band shirt order form and return it, along with payment, to school with your student beginning the first day of school.  The Band Shirt order deadline is August 29, 2025
