3.3 Changes and modification requests

Evolution of practices (research and clinical) requires a fluid documentation process with input from the clinical side. The gform below can be used by GO clinicians and staff as well as EBMA personnel to initiate a request to modify the documentation. The request will be logged and tracked to a task for completion by EBMA. The person requesting the update will be notified upon completion of form as well as the modification.
