A Physical Exam is required upon original entry into EPSD and in 6th grade.
**The exam must be completed after July 2019**
A Private Exam or School Exam will be accepted. Please notify me if you wish to have a School Physical Exam.
Dental Exam

A Dental Exam is required upon original entry into EPSD and in 7th grade.

Immunizations

Click Here to see the list of Immunizations required to enter school for your son/ daughter. Upon entry to EPSD schools your child's immunization record MUST be handed in and up- to- date.
7th grade students are required to have 1 Tdap and 1 Menningococcal Vaccine.
**If your child is in need of any immunizations, and you are unable to obtain the immunization from your health care provider, you may contact the Department of Health in Whitehall at 610-821-6770 for information and an appointment. **
