Bullying Incident Reporting Form
This form is for reporting incidents of bullying that have taken place at school or during school events. It may be filled out by the target of the act or by anyone who witnessed the incident(s). Please print it and bring it to a counselor or administrator.
If you are being physically threatened or there is an immediate safety concern, please contact your school's administration or call 911. You can also contact Baystate Crisis at (413) 733-6661.
This form is for reporting incidents of hate or bias that have taken place at school or during school events. It may be filled out by the target of the act or by anyone who witnessed the incident. Hate/bias incidents are acts of (micro-/macro-)aggression towards someone based on their perceived membership in a group by race, sex, gender, sexuality, ethnicity, or other protected class. It is not intended to be used if a student is in physical danger or in a mental health crisis.
You may instead submit a written report. Please return the paper form to the box in guidance.
