As stipulated in Section 3 Letter B of RA 11310, Case Management refers to a process used by the DSWD to enable the household beneficiaries to improve their functioning by dealing with their difficulties specifically in complying with the terms of the program. It shall refer to a mutually agreed process of assessing, planning, managing, coordinating and advocating for services and other interventions towards improving the well-being of households using the Social Welfare and Development Indicator (SWDI) and other-related case management tools.
The 4Ps KILOS UNLAD FRAMEWORK aims to guide the process of shepherding Pantawid Pamilya HHs to achieve improved well-being toward stepping-up and out of poverty; the numbers in the diagram represent the social case management components.Ā
STEP 1: ENROLLMENT - Registration of households shall be conducted during community assemblies/house-to-house visit to gather initial information on the familyās current socio-economic conditions, health, safety and protection concerns that may require immediate intervention and referral.

STEP 2: ASSESS AND ANALYZE - The case manager shall conduct a purposive home visit and family dialogue with the HH beneficiaries to conduct re-assessment of the familyās risks and vulnerabilities using CM Form no. 3. The information gathered will serve as a check and balance tool, along with the results of the SWDI, to identify needs and problems, strengths, and resources available as bases for crafting the household intervention plan.

STEP 1: IMPLEMENTATION OF THE HOUSEHOLD INTERVENTION PLAN - The case manager shall ensure sets of intervention such as butĀ not limited to cash transfers, FDS/YDS, health, social services, psychosocial services, education and training, labor and employment, livelihood and enterprise to ensure continued compliance to program conditionality and address the gaps towards achieving the beneficiariesā improved level of well-being. Ensure that enabling mechanisms such as access to resources, skills building, among others, are in place to facilitate effective service delivery in partnership and cooperation with the social service workforce. With the support of the social service workforce, the beneficiaries shall govern and initiate the fulfillment of their goals utilizing a task-oriented and problem-solving approach to access and avail resources geared towards moving-up to the transition stage. Lastly, the case manager shall monitor the services and interventions being provided throughout the implementation phase and respond promptly to changes in client needs and help the client alter services accordingly.

STEP 2: INTEGRATED DELIVERY OF SOCIAL SERVICES AND PROGRAMS BY THE SOCIAL SERVICE WORKFORCE/ REFERRALS - The referral is an encompassing task in the KU process since households may encounter special and difficult situations at any given time.Ā Thus, employment of immediate response and attention shall take place. The case manager must be prepared to refer them to the appropriate social service workforce to secure their safety and protection. It includes referring HH cases to members of the social service workforce for services such as psychosocial interventions (counseling, therapy, and psychiatric help); social welfare assistance (burial, transportation, medical, etc.); medico-legal and litigation services; and, protective custody, among others. The case manager, in consultation with the household, shall be guided by the case referral pathwaysĀ to link households to appropriate interventions.

STEP 1: TRANSITION OF SELF-SUFFICIENT HOUSEHOLDS - Transition refers to the last phase of the KU process specific to the planning for the future and building self-reliance and independence skills of eligible HHs as preparation for program exit. The beneficiary households who shall pass the transition eligibility criteria shall be deemed candidates for exiting.Ā To determine their eligibility, the case manager shall conduct a purposive visit and transition assessment to self-sufficient HH-beneficiaries (for two [2] consecutive periods) and listed as non-poor by the Listahanan. Household transition plans shall be developed by the case manager in collaboration with the graduation candidates and assigned LGU Link, tailor-fit to the circumstances of the HH.

STEP 2: NETWORKING AND CONVERGENT COMPLEMENTATION OF SERVICES - Self-sufficient HH-beneficiaries, either on their own or through their organized communities, shall be provided with additional opportunities to sustain their efforts and initiatives. This shall be evident through the localization of 4Ps Law through ordinance/resolutions, the inclusion of the 4Ps agenda in local development plans, provision of funds/grants to sustain community initiatives, and markets open to hiring 4Ps beneficiaries. Case Managers, in collaboration with other agencies, will lobby for sustainability support for the 4Ps beneficiaries. SWO IIIs and IPDO focal persons shall provide necessary assistance to case managers to sustain these efforts.

STEP 3: GRADUATION/EXITING - Prior to enrollment to graduation to exit procedures, HHs should have achieved economic and social advancement as measured by the transition eligibility criteria. Other conditions for the termination of program membership are as follows:
1. The HH transition planās goals and objectives have been successfully achieved; and,
2. The local government unit (LGU) is informed, ready, and equipped to provide continuing support services to its graduating constituents.
In the selection of the beneficiaries who have moved up to self- sufficiency level for the transition towards the Exit, there are three (3) main tools being utilized, the Listahanan, the SWDI, and the Case Management Report.
STEP 3: GRADUATION/EXITING (Continuation) - The beneficiary-households which possess at least two (2) the following eligibility requirements will be considered as candidates for exit:Ā
Assessed as Level 3 ā Self-Sufficiency in the current/ most recent SWDI assessment cycle;
Assessed as Level 3 ā Self Sufficiency in the preceding SWDI assessment cycle;
Assessed as non-poor in the most recent Listahanan;
Assessed to be prepared to exit from the program by the case manager/ CML using the transition assessment tool.

1) Terminal Report/ Case Endorsement Report (refer to CM Form no. 17),
2) CM Form 2 - Sustainability PlanĀ
3) Certificate of Improved Well-being;
4) Accomplished Transition Plan; and,
5) MOU with partner LGUs.
Without prejudice to Section 16 Rule IX of Case Management and Non-Compliance with Conditions, a qualified household-beneficiary shall be deemed to exit from the Program, WHICHEVER COMES FIRST, when:
The last monitored child in the household turns 19 years old;
The last monitored child in the household finishes high school;
The household reaches the 7-year duration in the Program;
The household is no longer poor, based on the latest assessment thru the adopted standardized targeting system;
The household voluntarily waives its membership from the Program; or
The household commits offenses wherein the sanction is delisting, subject to the standards to be developed by DSWD.
