•                  Duly accomplished Application Form

•        PSA issued Birth Certificate of Minor

•                 Photocopy of PSA-issued Marriage Contract of minor's parents. /    Copy of Court issued  Legal Guardianship / Tallaq or Fasakh          Certification from Shariah Court or any Muslim Barangay or religious  leader.

•                 PSA issued Certificate of No Marriage (CENOMAR) for non-marital child

•                 Notarized Affidavit of Consent signed by both parents if married, mother only if unmarried

Additional Information for the Affidavit of Consent (Place of Destination, Name of Travel Companion, Relationship to the Minor, and Purpose of Travel e.g Vacation)

•                Notarized Affidavit of Support and proof of Financial Capacity, such as       (Certificate of Employment and or Latest Income TAX Return or Bank       Statement

•               Photocopy of 1 valid Identification Card with 3 specimen signature of       parents, Mother only if not married

•               2 pcs Passport-size I.D picture of the minor (White Background within                6 months)

•               Waiver of Parents
