Certificate of Acceptance / Enrollment / Registration where the minor intend to study

B. For Minors Attending Conference / Competition / Educational Trips / Other Related international Activities

1. Certification from sponsoring / inviting organization

2. ( For sports competition ) Affidavit of Undertaking indicating safety measures undertaken by the sports agency

3. Invitation from the Sponsoring / Inviting Organization with itinerary and list of participants and duration of the travel / activity

4. Assessment report from the local Government Unit where the minor resides ensuring that the competition is not harmful to the health and development of the minor.

C. For Medical Purpose

1. Medical Abstract

2. Recommendation from attending physician

D. For Inter-Country Adoption

Notarized Affidavit of Undertaking by Foster Parents
