List of Requirements 

1. Duly accomplished Application Form

2. PSA Authenticated Birth Certificate of Minor ( Photocopy)

3. (For Married) - PSA Authenticated Marriage Contract of Parents (Photocopy)/Court Decision on  Legal guardianship/Tallq or Fasakh Certification from the Shariah Court or any Muslim barangay or religious leader.

4. (Not Married) - PSA Authenticated Certificate of None Marriage (CENOMAR) of minor's mother with attached Solo Parent Identification (ID)/Certification from the Local Government Unit (LGU)

5. Duly Notarized/Authenticated Affidavit of Consent of both parents / Solo Parent / Legal Guardian

• Affidavit of Consent and Support from parents (if married); or 

• Affidavit of Consent and Support from mother only (if not married)

• Notarized consent and support should be based on the present address of the parents.

• Kindly indicate on the affidavit: Place of destination, Travel Companion, relationship, and purpose of travel.

6. Photocopy of 1 valid ID with 3 specimen signatures of Parent/s

7. Duly Notarized  / Authenticated Affidavit of Support and copy of Proof of Financial Capacity, such as:

(Certificate of Employment; or Latest Income Tax Return; or Bank Statement)

8. 2 pcs. Passport -sized ID picture of Minor

(White background with collar and taken within 6 months) 

9. Passport of Travel companion (Photocopy)

10. Death Certificate for (Deceased Parents)

11. Old Travel clearance (Original for Renewal)

12. Php 300.00 / child processing fee (Valid for 1 year) or Php 600.00 processing fee (Valid for 2 years)
