These Items are Required for Student Licensing Files
(Items 1&2 MUST be submitted BEFORE a child can start):
Parent/Guardian Information and Permissions Packet
(pages 3-8)
Immunization Record (Proof of immunization or exemption)
Special Diet and/or Health Care Plan, if applicable
The Child’s Statement of Health Status for Enrollment
(within 30 days of enrollment) (pg. 10)
Requirements for Student Files
Please note that there can be no blank spaces. If there is no information available, please write “None,” as “N/A” is an unacceptable response.
The Child's Statement of Health Status for Enrollment MUST include the "Next Date Visit is Required" and MUST be signed by a licensed physician or licensed nurse practitioner.
There must be specific information listed for all Emergency Medical Authorizations.
Please ensure that electronic Parent/Guardian signatures are properly completed, as follows:

We would like to periodically include student photos in our class updates. In order to do so we need your consent for your child to appear in these photos. Please fill out and return this Permission and Release form with your child's licensing documents.

The ECE student sign-in/out form is required for all ECE classrooms. A legal document required by State Child Care Licensing, for security purposes this form must be maintained daily by the center.
DPS ECE classrooms will only release children to an individual over the age of eighteen (18) for whom written authorization has been given by the parent(s)/guardian (s) and is maintained in the child's record.
The sign in/out sheet must include for each child:
the date
the child’s name
the time when the child arrived at and left the center
the parent or guardian’s FULL signature (first and last name) or other identifier for both sign in and sign out
