Requests for excused absences will be managed by the Associate Dean’s Office. Students must submit a Letter of Excuse addressed to Dr. Aireen B. Arnuco. For undergraduate students, this letter should be signed by both the student and a parent or guardian.
The Letter of Excuse must be accompanied by a medical certificate and any related medical documents, which will be reviewed by the DLSU Health Service Office (HSO). A Yellow Slip from the HSO is also required to accompany the Letter of Excuse.
For absences due to mental health conditions, a clearance from a psychiatrist or psychologist must be included. This clearance will be assessed by the Office of Counseling and Career Services (OCCS).
If the absence is due to participation in conferences, seminars, or competitions, the Letter of Excuse should include all relevant documents related to the activity, with proper endorsements. These documents must be submitted along with the letter to the Associate Dean.
Following pages 98-99 of the 2021-2025 DLSU Student Handbook, absence due to the following reasons can be approved by the concerned faculty members:
2.1 Death of an immediate family member;
2.2 Emergencies;
2.3 Caregiving of sick family members;
2.4 Internet issues;
2.5 Sickness that does not usually require a medical certificate;
2.6 Delayed submission for course requirements;
2.7 Other special cases not mentioned above.
Students are reminded to process their request 7 days after returning to their classes. Late requests will not be accommodated.
