5th Grade Ensemble Registration
Please fill out the information below to register for band, orchestra or choir for the 2026-2027 school year. By filling out this form you will be enrolled in the ensemble of your choice. After you complete this form please secure an instrument for band or orchestra. You may visit www.qandf.com, or call 800-637-6872, to rent an instrument and get the necessary supplies for this year.
