Sports Physical
Sports Physical (Middle School Only): Students must have a current sports physical to participate in interscholastic sports. The physical must have been performed within 13 months of the start of the specific sport. A sports physical must be turned in to the health office BEFORE trying out for any sport and must be active for the full sports season. Please be sure to have a parent complete the first page!
An annual physical form can also count as a sports physical and moving forward will be valid for 13 months after the exam date.
The middle school nurses encourage students to stop by the health office to see when their physical expires prior to attending tryouts!
