Sports Physical Form

Welcome Back to School!

It's hard to believe that summer is just about over and another school year is about to begin! Students who require medications at school can drop off the proper forms and medications in their original containers beginning the week of August 13th to the main office. If you have any questions on forms that are needed, or school protocols regarding a health condition, please never hesitate to send me an email or navigate through the health office website.

For those middle school students who plan on participating in a sport, a sports physical must be on file in the health office prior to participation. Please send me an email asking if your physical from the previous year is still valid. If you have gotten a new physical over the summer, it can be dropped off to the main office anytime.

Cheers to a happy and healthy school year!

My Background

As the oldest of nearly 40 grandchildren, I have always been surrounded by kids and have enjoyed taking the leadership role. This is what lead me into the education field while at DePaul University. I graduated in 2006 with a Bachelors in Elementary Education and began my career as a middle school science teacher. I loved working with students everyday and enjoyed seeing "the light go off" as they began to figure things out with my guidance. After leaving teaching to begin a family, I quickly realized I was ready to get back to work. Unsure if teaching was where I wanted to return, I decided to give nursing a try. In 2014 I graduated with a Masters in Nursing from Rush University. Quickly realizing that I missed the school setting, I left hospital life to get back to the kids I missed working with everyday. I joined District 58 in 2016 and have found a home here!

As the school nurse for both Pierce Downer Elementary and Herrick Middle School, I strive to provide competent and evidenced-based healthcare to anyone entering these facilities. A core principle I carry is that all individuals need to take an independent role in their own health. Whether you have a peanut allergy or have been diagnosed with diabetes, students need to understand how to effectively manage these conditions in the real world. My role is to educate students on how handle real life situations and guide them on how to make smart healthy choices each day.
