prepare the District approved documents (SIP) in:
electronic copy (PDF) and
1 printed copy
Submit the PDF file by filling out the form below;
Screen capture/print the submission receipt sent to your email;
Submit the printed copies to records section;.
Note: Present your submission receipt to Records Section when submitting hardcopies of your SIP.
Check or Monitor status of your submission through the status sheet below.Â
