Documents for Submission

• Original copy of School Form 9/ Class Card (Last Grade Level Attended Report Card);

• National Statistics Office (NSO) Philippine Statistics Authority/ (PSA) copy of Birth Certificate; Original copy of Certificate of Good Moral Character from the Principal, Guidance Counselor, or Class Adviser: (for Grade 7 and 11).

• Two (2) pieces of 1" x 1" identification pictures; Enrollment Application Form.
