Download the Enrollment Form (AF2)
Go to the official website.
Locate the Enrollment Form (AF2) and click Download to save it to your device.
Fill Out the Form with Correct Information
Open the downloaded AF2 file (PDF or Word format).

Scan the Documents
Once completed, sign the form if needed.

Save the scanned file in PDF format, with a clear and readable filename (e.g., "AF2_John_Doe.pdf").
