INSTRUCTIONS
1. Please download the reporting cover sheet and save as an Excel document using your Organization Name as the file name before uploading to Submittable. To download, click “File,” select "Download as," and select Microsoft Excel file.
2. Only include donations received from Nov 1 to Dec 31, 2022.
2a. If a donor gives more than the maximum of $1,000, enter the full donation in Column D "Total Amount of Donation" and the eligible amount, up to the maximum $1,000 per donor, in Column E "Amount Eligible for PowerProgress".
2b. If a donor set up a new recurring donation during PowerProgress, enter the amount of the recurring donation in Column D "Total Amount of Donation" and the annual value of the recurring donation, up to the maximum $1,000 per donor, in Column E "Amount Eligible for PowerProgress." Enter “Yes” in Column F. Please make sure to list new recurring donations only once, even if they recurred more than once between Nov. 1 and Dec. 31.
3. In addition to completing this Match Cover Sheet Template, please prepare a PDF or Excel document "Match Documentation" showing online donation records, copies of checks, bank statements, and other records that can be used to verify donations.
3b. If you are reporting donations via online online donation processor like ActBlue or a CRM like Salesforce you can submit an Excel export of donation records instead of or in additional to a PDF document.
Note: You may conceal bank account details on checks you submit as documentation.
After the match ends, you will have until January 31st to submit your post-match report and documentation to JustFund. Please submit your match documents directly to Jessie Hankins: Jessie@democratizingphilanthropy.org.
Once you submit your documentation and Cover Sheet, (and someone from your org completes the closing survey) your matching grants should hit your accounts through JustFund within two weeks.
