Final Forms/Physicals/Pay to Play

All athletes need these three things:

Physical Form: Please click here to find the most up-to-date physical form for your athlete. You can turn in your athlete's physical to the Dempsey front office, our Athletic Director, or either coach on the first day of practice. **ALL RUNNERS MUST HAVE ALL THEIR FORMS TURNED IN BEFORE THEY CAN ATTEND PRACTICE**.
Final Forms Sign-up: All runners should sign up for Track in Final Forms. Please click here to see directions on how to complete this process. Please let us know if you have any questions about this process (harryme@delawarecityschools.net) or (kunoka@delawarecityschools.net).
Pay to Play: Pay $25.00 School Sanctioned Sport Pay-to-Participate Fee (payable to Delaware City Schools). This can be give to the front office, our Athletic Director, or coaches.

Uniform Order Form

Dempsey provides all athletes with a shirt for the track season. It will need to be WASHED and RETURNED to a coach at the end of the season. Please use this form to request your shirt size. PLEASE NOTE THAT ALL UNIFORMS ARE ADULT SIZES. Athletes will need to purchase a pair of ALL black shorts.
