Transportation & Media Release Forms

 All Graduating Seniors, who plan to walk in the May graduation ceremony MUST have a parent or guardian sign the combined permission forms found below. The TRANSPORTATION FORM and MEDIA RELEASE FORM are written in English and Spanish. The forms can be completed on smartphones, tablets, or computers.
