Working Papers

In accordance with State Guidelines, the Guidance Department will continue to process requests for working papers. After you have completed the steps listed below, please bring your completed application to the DPHS guidance office.

 

Students age 14 but under the age of 18 wishing to obtain employment must complete an Application for Employment Certificate.  Successful completion of this document will result in students being issued a Working Card, which identifies that they are eligible to hold an after school job.  

 

Steps to obtaining a Working Card:

1.Complete "PART I- Parental Consent" on the Application for Employment Certificate.

2. Have the "Physical Fitness Certification" page completed by your pediatrician or, for student athletes, the high school nurse.

3. Turn in both signed documents to the Guidance Secretaries.

4. Guidance Secretaries will typically create the valid working card within 24 hours of receipt of signed documents.

Things to remember:

There are TWO (2) types of working cards- one for students age 14 & 15, and one for students age 16 & 17.

If a student is issued a working card at the age of 15 they MUST complete a new Application of Employment Certificate when they turn 16.
