Step 1: Read and understand the 2024-2025 Parent Handbook that is attached below prior to registration.
Step 2: Register your child online using the following link.
Step 3: Complete the necessary forms (See uses of forms below to determine need) and return to a PCE BASE staff member.
Medication Release Form: For children who receive medication during program hours. Must be signed by Primary Care Physician.
Extra-curricular Release Form: For children who participate in extra-curricular activities while attending the program. Ex: Band, orchestra, etc.
*Please allow a minimum of 48 hours for PCE BASE to approve enrollment.
