*This form must be filled out and your MySchoolBucks CO All State Application Fee must be submitted by September 4, 2025. Failure to submit the form and payment by the due date will result in disqualification from the CO All State Choir audition.

*Please note that audition fees are non-refundable, regardless of the audition outcome, missed auditions, or withdrawal from the process.

If you are selected for the CO All-State Choir then you must complete a second round audition on February 5, 2025. You music learn and memorize all of the required music for the choir you are placed in.
