Parents choosing to involve their child in school or community-sponsored activities (tutoring, girl/boy scouts, band, choir, tennis, science clubs, etc.) during program hours shall complete the Extra Curricular Form (above). When children transition to and from school or community-sponsored activities, children shall sign into the B.A.S.E. program before transitioning to and from and before or after school activities. Staff shall check children in and out of the program as part of their attendance procedures.
For staff to administer over-the-counter (fever reducers such as Tylenol, ointments, and creams used as a treatment for a skin condition, eye or ear drops) and prescription medication, the following is required

We schedule swimming activities as part of our program. To ensure your child’s safety, we are requesting information about your child’s swimming ability. Please place your initials next to the level of swimming listed below that coincides with your child’s current ability to swim.
The policy regarding underage individuals picking up children from BASE was amended in the 22 - 23 school year. The new policy, which complies with state regulations, will allow persons ages 16 and older to sign out younger siblings, neighbors, etc. from BASE, as long as parents have submitted and signed the required form.
