Daemen University is happy to thank you for your investment in the education and
training of our students with a $750 honorarium per 4-week clinical clerkship, per student.
Complete the following steps to receive your honorarium:
Complete a W9 form. This only needs to be completed once.
Upload your W9 to the Daemen University's Secure Portal HERE.
PLEASE DO NOT EMAIL, FAX OR MAIL ANY TAX INFORMATION, INCLUDING W9 FORMS,
THAT MAY CONTAIN PERSONAL IDENTIFYING INFORMATION,
THEY MUST BE SUBMITTED THROUGH THE PORTAL ONLY
The honorarium will be processed monthly.
