                              Sport Physical 

If your student plans to try-out for a sport next year. Please do not delay, complete a sports physical over the summer. Students are not allowed to try-out for a sport without the completed sport physical. 

                            Medication at School

If you want your student to receive medication during the school year. Please complete the d300 medication form. There is a parent portion and a doctor portion. Medication needs to be dropped off by an adult, with the completed form. This includes, Advil, Tylenol, any otc medication.

                                                        6th Grade Dental Exam

All 6th grade students are required to submit a completed dental exam. The dental exam must be completed after October 15, 2024. This is due to the nurse's office by May 15, 2026.   

                                                                 Medication drop off

ALL medications that students will be taking at school need to brought to school by an parent or an adult. Students are not permitted to carry medication with them. If you have any questions, please call the nurse's office.
