Service/Leadership Form Directions

• DO NOT ALTER THE FORMS IN ANY WAY

• at least 10 cumulative hours (not annually) per activity

• 1 PAGE PER ACTIVITY (You may have multiple examples of each)

• only include HIGH SCHOOL volunteer service hours – no pre-high school hours

• AVOID acronyms

• Activities and experiences may be inside or outside of Grayslake North

• Phone number, Email, and Name, is necessary to verify service and leadership

• Service Activities and Leadership forms completed by parents are NOT ADVISED

• Advisors may verify participation in the activities listed on the application and any erroneous or inaccurate information will disqualify you from admittance into NHS.

• Thoroughly explain each activity in the space provided. Be as detailed and specific as possible in order to fully represent your work.
